Limit one (1) free Code request per postmarked outer envelope. One (1) unique Code and one (1) UPC barcode will be sent in response to each request. No mass or mechanically reproduced requests will be accepted. All requests become the sole property of the Sponsor. Sponsor is not responsible for lost, late, stolen, misdirected, damaged, incomplete, illegible or postage-due mail or requests. Each Code request will be mailed separately within approximately seven (7) days. Upon receipt of your free Code, visit the Web Site before 11:59:59 a.m. ET on 10/18/10 and follow the instructions on screen to create your entry account by providing all required information as stated above and submit your entry. All entrants must have a valid e-mail address in order to participate in this Game. Each Code is unique and is good for one (1) Game play during the Promotion Period. You may come back each day for another chance to play the Game by entering a Code and UPC barcode. A "day" is defined as the twenty-four (24) hour period beginning at 12:00:01 a.m. ET and ending 11:59:59 p.m. ET except on the first day of the Game (6/21/10) when the day will begin at 12:00:01 p.m. ET and end at 11:59:59 p.m. ET and on the last day of the Game (10/18/10) when the day will begin at 12:00:01 a.m. ET and end at 11:59:59 a.m. ET. In case of a dispute over an online entrant’s identity, the Authorized Account Holder of the e-mail address used to enter will be deemed to be the entrant. In the event the Game is terminated prematurely and subject to the limits established in these full Official Rules, all eligible entries received prior to termination will be eligible for the Second Chance Drawing. Sponsor is not responsible for printing or typographical errors in any Game-related materials.   FOURTH PRIZES (4,849): Four thousand eight hundred forty-nine (4,849) Fourth Prize winners will each receive a package of Playtex Gentle Glide Tampons awarded in the form of a VIP coupon, good for one (1) package, up to $8.99. ARV of each Fourth Prize: eight dollars and ninety-nine cents ($8.99). ARV of all Fourth Prizes: forty-three thousand five hundred ninety-two dollars and fifty-one cents USD ($43,592.51). Estimated odds of winning 1:2103.

  Use of the Visa gift cards are subject to applicable policies, terms and conditions imposed by the issuer. Total ARV of All Prizes: fifty-five thousand four hundred sixty-seven dollars and fifty-one cents USD ($55,467.51). Estimated odds of winning any prize: 1:2,039. Odds stated for all prizes are approximate, actual odds of winning depend upon the total number of valid Game plays completed during the Promotion Period

• GENERAL PRIZE CONDITIONS /PRIZE CLAIMS: The Game results will be instantly displayed at time of Game play if you are a potential winner. Potential winners will be provided with directions for prize validation and how to claim his/her prize on screen. You are not a winner of any prize unless and until your eligibility is confirmed. Potential Grand Prize winner will be notified at the phone number submitted on the online entry account and will be required to complete and return an Affidavit of Eligibility and Liability/Publicity Release without additional compensation, where permitted. All First, Second and Third Prize winners may be required to complete an address confirmation letter as a condition of receiving a prize. Documents must be returned by the means specified in the specific document within the time specified or the potential prizewinner will be disqualified. All claimed prizes by persons who are found eligible will be awarded to those persons. No substitution, assignment or transfer of any prizes will be permitted, except at the sole discretion of Sponsor, who reserves the right to substitute a prize or prize component with cash or another prize of comparable or greater value. Grand Prize will be awarded at the end of the Promotion Period following winner validation. All First, Second, Third and Fourth prizes will be processed monthly throughout the Promotion Period within approximately 2 weeks of the end of each calendar month, following winner validation. Fourth Prizes will be mailed to the mailing address submitted on the entry account. Please allow approximately 6–8 weeks for delivery of prizes. All federal, state and local taxes are the sole responsibility of each prizewinner. A 1099 form will be issued to any winner (or parent or legal guardian if winner is an eligible minor) who receives a prize with a value of $600 or more. In the event that a potential winner is considered a minor in his/her state of residence but is otherwise eligible to participate in this Game, the prize will be awarded to the parent/legal guardian on behalf of such minor, who must agree to execute all required documents. • SECOND CHANCE DRAWING: In the event that any Grand, First, Second or Third prize is unclaimed or unawarded after the end of the Game, Sponsor will conduct a random drawing from all eligible entrants who played the Game during the Promotion Period and who did not otherwise win a prize during the Promotion Period. All registered eligible entrants will receive one (1) entry into the Second Chance Drawing, regardless of the number of times the entrant participated. The Second Chance Drawing will be held on or about 11/1/10. The odds of winning any prize in the Second Chance Drawing depend on the prizes available and the number of eligible entries received for this Second Chance Drawing. Prizes will be awarded in the same manner set forth above. If any prizes are still unclaimed or returned as undeliverable, those prizes will not be awarded. All potential winners are subject to verification for compliance with these full Official Rules.
• SPECIAL GAME LIMITATION: Any attempts by an individual to access the site via a bot script or other brute-force attack will result in that e-mail address becoming ineligible. Sponsor, in its sole discretion, reserves the right to cancel or modify the Game without notice or liability if Web Site becomes infected with a virus, or is technically corrupt, or if the Game otherwise cannot operate as planned. Sponsor and its affiliates, subsidiaries, parent company and agencies assume no responsibility for any loss or damage to entrants’ or to any other person’s computer relating to, or resulting from, entering or downloading any information or software in connection with the Game. Sponsor and its agents are not responsible for technical, hardware, software or telephone/Internet malfunctions of any kind, lost or unavailable network connections, or failed, incorrect, incomplete, inaccurate, garbled or delayed electronic communications caused by the user, or by any of the equipment or programming associated with or utilized in the Game, or by any human or other errors that may occur in connection with the Game. If any of the foregoing malfunctions, errors, fraud, unauthorized intervention or defects render the Game incapable of operating as planned or corrupt the integrity, affect the administration, security, or fairness of the Game, Sponsor, at its sole discretion, may suspend or cancel the Game immediately. Under no circumstances will more prizes than those set forth in these full Official Rules be awarded. If a technical error or the like occurs that results in more potential winners than available prizes for a particular prize level, Sponsor may elect to hold a random drawing from all eligible claims received to award the number of prizes for the level of prize in dispute which have not yet been awarded. Any use of micro, automatic, programmed, robotic, or like entry methods will void all such entries by such methods, and entrants using such methods will be disqualified. Sponsor further reserves the right to disqualify any individual who tampers with the entry process. Sponsor may prohibit an entrant from participating in the Game if it determines that said entrant is attempting to undermine the legitimate operation of the Game by cheating, hacking, deception or other unfair playing practices or intending to abuse, threaten or harass other entrants. Any attempt by an entrant or any other individual to deliberately damage, interfere with, or undermine the legitimate operation of this Game is a violation of criminal and/or civil laws, and should such an attempt be made, Sponsor reserves the right to seek damages and other remedies from any such person to the fullest extent permitted by law. • ELIGIBILITY: To participate in the Game, entrants must be legal U.S. residents currently living in the 50 United States or DC’ eighteen (18) years of age or older (nineteen (19) or older if a resident of AL or NE, twenty-one (21) or older in MS), and have Internet access as of 6/21/10. Void where prohibited. Employees, officers and directors of the Sponsor, its parent companies, affiliates, subsidiaries, prize suppliers, advertising, promotion and fulfillment agencies, and members of the immediate families (defined for these purposes as including spouse, biological and stepparents, children, siblings, grandparents and grandchildren, and the respective spouses of any of the foregoing, regardless of where they reside) or households (whether related or not) of any of the above, are NOT eligible to participate.
• RELEASE OF LIABILITY: Except where prohibited, by participating in the Game, you agree to release and hold harmless the Sponsor and each of their respective parent, subsidiary and affiliated companies and each of their respective officers, directors, employees, agents, licensees, and assigns, and sponsors, advertisers, partners and agencies from and against any claim or cause of action arising out of participation in the Game or receipt or use of any prize, including, but not limited to: (a) unauthorized human intervention in the Game; (b) technical errors or interruptions related to computers, software, servers, or telephone or network lines; (c) printing or typographical errors; (d) errors in the administration of the Game or the processing of entries; (e) late, lost or undeliverable mail, e-mail or shipments; or (f) injury or damage to persons or property that may be caused, directly or indirectly, in whole or in part, from your participation in the Game, or receipt or use of a prize.
